Trained as a rural family physician, Dr. Reitmeier’s diverse medical background includes experience in emergency care, inpatient treatment, surgical assistance, and obstetrics—demonstrating his versatile approach to medicine. Driven by a passion for dermatology, he earned both a Postgraduate Diploma and a Master’s degree in Practical Dermatology, solidifying his advanced knowledge and skills in the field.

Dr. Reitmeier’s current practice reflects a special interest in medical aesthetics, practical dermatology, sexuality, and gender-affirming care. He sees patients at two clinic locations: Form Medical Aesthetics in Winnipeg and ReitMD in Portage la Prairie. As a Distinguished Alumnus of the University of Manitoba and a member of the College of Family Physicians of Canada, he has earned national recognition for his contributions to medicine.

With a background in psychology and progressive leadership experience in healthcare, Jake brings a strong operational focus paired with a deep commitment to patient experience and staff development.

He has also been instrumental in the delivery of EXOMIND™, a technology used for mental wellness and brain optimization offered at Form Medical Aesthetics, ensuring its integration into patient care with a focus on accessibility, innovation, and safety.
